When the goals of treatment with an agent are not achieved, the physician may titrate the dosage, change the therapeutic agent, or add or remove medications. The effectiveness of the therapy should be reevaluated every three months and should include A1C testing, self-monitoring of blood glucose, incidences of hypoglycemia, psychosocial situation and adverse events. When more than one medication is being used it is important to choose agents that have complementary mechanisms of action.
